Dear Enterprise Zone Community,
On behalf of the Department of Housing and Community Development, I am releasing the new voucher forms that you should use to either request or issue a voucher certificate. I have attached both the forms as well as the guidelines for their implementation.
Generally, you should plan on implementing the new forms as of January 1, 2008. However, as the guidelines note, we encourage both the enterprise zones and the businesses to begin using the forms as soon as possible. Also, please note the interim rules that request that the enterprise zones and the businesses observe the requirements presented in the applications. These rules apply if alternative forms are used in the interim period leading up the required effective date, January 1, 2008.
You will note that the attached files for the forms are in a WORD format that you can complete online. Just as importantly, you can save the files. However, if you prefer your files in the regular WORD format, we will post these versions on our Website in the next few days.
